PROVIDER APPLICATION
This application is for anyone interested in becoming a Lifesharing Provider with RHD. Please click the button "Provider Application" to go to our online application form. Complete all highlighted and required areas. Once you submit, click the link sent to your email to verify your identity, and your application will automatically be sent to us. Someone will be in-touch within a week. If you have questions or want to check the status of your application, call 484-298-1333.
*Please note that Lifesharing Providers are Independent Contractors, and in accordance with RHD policy, active RHD employees are unable to apply.*
CRIMINAL
BACKGROUND CLEARANCE
This is the document for a criminal background check. Checks are completed bi-annually (every 2 years) for existing providers and family members living in the home over 18 years of age or within 90 days of your application for new providers. Please download, complete, and email to your designated lifesharing specialist or NEPAlifesharingproviders@rhd.org
PROVIDER (and family)
PHYSICAL FORM
Physicals & TB tests are completed bi-annually (every 2 years) for existing providers and family members living in the home or within 90 days of your application for new providers. Please download, have your physician complete, and return to your lifesharing specialist or NEPAlifesharingproviders@rhd.org

SAFETY RECORD
Driver safety record checks are completed during the application process for new providers. Please download, complete, and email to NEPAlifesharingproviders@rhd.org
